GUIDELINES FOR ISSUANCE OF VISA TO POLAND
A visa is issued in accordance with the length and purpose of visit. The Embassy reserves its right to request for additional documents. The general requirements are listed hereunder.
I. TOURIST / VISIT / BUSINESS / CONFERENCE / STUDY / SCIENTIFIC & CULTURAL
A. FILIPINO /ROC /CANADA
1. One duly completed TYPEWRITTEN application form
2. One (1) recent passport-sized picture
3. Original passport (valid at least 6 months prior to departure)
4. Business Name and/or Certificate of Employment (specifying position, date of employment, salary) and Approved Leave of Absence
5. Latest Income Tax Return
6. Bank Certification / Bank Statement (last 6 months) and/or Bank Passbook.
7. Health Insurance Certificate / Travel Insurance Certificate.
8. Flight Confirmation & Itinerary
9. Hotel Confirmation Certificate
10. For group tour / travel agency: letter from the travel agency specifying list of passenger, tour program and name of partner agency / co-organizer in Poland.
11. In case of visit to a relative or friend by personal invitation: must secure official invitation “ZAPROSZENIE” issued by the governor’s office (VOIVODSHIP) in Poland.
12. For sponsored trips: Invitation Letter from the Host person / company in Poland; Bank Statement (last 6 months) of the company sponsoring the trip; Request Letter from the sponsor company specifying name/s of traveler/s, purpose of the trip, also stating support and / or guarantee of all travel expenses.
B. PROC
Telex approval is required, which takes approximately 1 week.

2. Request Letter and Guarantee from Shipping Agency in Poland

III. TRANSIT VISA (stay at airport transit area only)
1. Duly Completed TYPEWRITTEN application form.

IV. WORKING PERMIT
1.Work Permit from Poland
